Reward Point Survey

Second to spam, Zynga loves surveys and will spam those any chance they can. I purchased some Reward Points today and got the option to take a survey about my experience.
I decided to check it out and here are the questions.

    I got a similar survey pop-up when doing NY jobs in my energy account.
